Stroke is a potentially severe complication of surgery encountered in post anaesthetic care units, surgical wards and intensive care units. The 30-day mortality after perioperative stroke is double that of community-based stroke. Survivors are at risk of considerable disability, which diminishes quality of life and creates an enormous economic burden. The estimated rates of perioperative stroke are between 0·1 and 0·8% in patients undergoing non-cardiovascular and non-neurological surgery, and 2% in patients undergoing cardiac surgery, although rates are likely higher in clinical practice. These clinically apparent strokes are but the ‘tip-of-the-iceberg’ with estimated rates of subclinical perioperative brain infarcts exceeding 60% of patients undergoing select procedures. Many of the clinical situations associated with perioperative stroke lack high-quality evidence. Future studies focussing on pre-operative risk stratification, intraoperative neuroprotective measures, and postoperative detection/therapeutic strategies will reduce the incidence of and improve outcomes following perioperative stroke.
