Lecture Summary

Sepsis survivors often face many physical, mental, and emotional challenges following hospitalization. Post-sepsis complications often lead to high rates of post-discharge mortality and readmissions. Evidence suggests that targeted multicomponent support during the transition out of the hospital can improve outcomes for sepsis survivors, but much is still to be learned.
