Lecture Summary

Urinary Tract Infections (UTI) are very common and evolving resistance patterns can impact treatment choices. Factors that influence management include distinction between cystitis (drugs that concentrate in urine) and pyelonephritis (requires adequate levels in kidney tissue) and uncomplicated vs. complicated infections that may require more broad-spectrum agents. Nitrofurantoin macrocrystals 100mg bid x5d should usually be first choice for cystitis. For pyelonephritis start with 1 time IV dose of ceftriaxone or gentamicin followed by oral antibiotics for outpatient management. Check for ureteral obstruction in patients with systemic signs of infection. Increasing antibiotic resistance including Extended Spectrum Beta-Lactamases (ESBLs) require knowledge of local resistance patterns for best antimicrobial choices.
